- FORMS- New Youth Application (print and fill out page 5), New Adult Application (print and fill out pages 4 & 5), Annual Medical & Health forms (Only one form effective January 1, 2010 - Parts A and C completed by Scout Parent or Adult - Part B medical by Physician - sorry attached physicals no longer permited on long term camps over 72 hours). (Campouts require a physical also but the Troop will accept attached physicals at this time or until council overrules this.)
